Step 01

Hair Sample Collection Instructions

Step 02

Print this:
Lab Requisition form

(complete and mail in with your hair sample)

Step 03

Schedule our ‘Results and Recommendations’ call

Be sure this is at least 2 weeks from date of purchase to allow lab time to process results. Session expires one month from date of purchase.

Step 04

Mail in your hair sample.

NOTE:  It takes up to 2 weeks for us to receive results after you’ve mailed in your sample, so please schedule this 3 weeks from today, if you plan to mail in your sample within the week.
