Lab Tests & Interpretations
It is common practice for naturopaths, nutritionists, and other non-licensed practitioners to collect your signature on a liability waiver form such as this. By checking the terms and condition box , you acknowledge that it is your responsibility to deliver all laboratory test results, now and in the future, to your own physician for any medical interpretation or opinion regarding any laboratory results provided by Diane Kazer, or her referring health practitioners. The undersigned agrees that he or she will receive a nutritional interpretation of the test results from Diane that is to be used exclusively by the undersigned as an educational tool for personal health purposes. However, the personal physician of the undersigned may use these same laboratory results to diagnose and treat disease. The information on Diane’s web sites, brochures, flyers, and information packets are believed to be extremely accurate, but such accuracy cannot be guaranteed by Diane, her independent representatives, associates and affiliates as we are not the originators of the underlying data used in the interpretation. The undersigned releases Diane and her affiliates from any liability for injury or loss arising out of the use of, or reliance on, the laboratory results and/or the dietary, supplement and lifestyle suggestions provided. Before making any changes to the exercise, diet or nutritional or hormonal supplementation of the undersigned, a physician should be consulted.
Diane Kazer does not diagnose, cure or treat any illness or disease. Out of reference laboratory reference range results will be indicated on the official lab result form provided by Diane Kazer from a State Certified Laboratory to the undersigned. This information is not intended to, cannot, and should not be expected to substitute for a personal consultation with your own physician. Further, the undersigned releases Diane Kazer, referring health practitioners, her lab partners, her independent representatives, associates and affiliates from any and all liability for any failure to identify any medical condition or disease. It is understood and agreed that this is not the purpose of their services.
If you make a mistake collecting saliva or urine specimens, etc., or if the lab refuses to accept specimens because of a mistake you made, a free replacement test kit will be provided to you. Please call the office before discarding any specimens collected. Otherwise, once test kits are in your possession, they may not be returned and no refunds are allowed for any test kits due to safety, hygiene and accuracy concerns.
Every effort has been made to keep costs down as low as possible for you! Because of this and the fee structure, there are NO REFUNDS issued on payments made to Diane for this partnership. Further, once the first payment has been made to Diane Kazer for partnership, the client agrees to pay for every month thereafter for the term. There will be a $50 fee for every payment more than 5 days late. Included in the price of each partnership is a $500 account set up fee.
All sales are final on digital programs. Digital programs come with lifetime access, unless otherwise indicated.

Supplement Returns/Refund Policy
All supplement sales are final, we are unable to offer refunds once the product leaves our warehouse.
If your package is returned to us without approval there will be a 20% restocking fee applied. We do not process refunds on shipping and handling.
If your order is lost in transit, please contact our support team at firstname.lastname@example.org with your order number and tracking information so that we can investigate the lost package and sent a replacement if necessary.
Any errors – including mis-shipments, missing items, damaged items, etc. must be reported within in 15 days of order delivery to receive a resolution or correction.
Please allow 1-3 business days for your order to ship, we will provide tracking information once your order has been processed so you'll know when to expect delivery.
Transit time typically takes anywhere from 3-10 business days depending on your location, and may take up to 20-30 days for international clients.
International customers, please note: Your local customs office may apply additional taxes and duties to your order once it arrives in your country. We recommend you check ahead with your local custom office to asses duties and fees before placing your order. We will not refund the cost of shipping and your order will be subject to a restocking fee if you cannot clear it through customs and it is returned back to us. Thank you for your understanding.
Expiration for Services
Expiration date for services are upon completion of program according to the terms listed in the Agreements for the corresponding program, agreed upon both parties.
